You will first need to contact either your GP or the Bereavement Office at the hospital to begin the process of obtaining the Medical Certificate. Once issued, the certificate will be forwarded to the Medical Examiner, who will contact you within 72 hours of receiving it.
After you have spoken with the Medical Examiner, the certificate will be sent to your local Register Office. A registrar will then contact you directly to arrange an appointment for you to register your loved one’s passing.
Once the registration has taken place, the registrar will provide you with copies of the death certificate. They will also either hand you the Green Form, which we require, or—more commonly—email it directly to us.
